Group Dental
- All of our Group Dental Plans are Guaranteed Issue - No Industry Limitations
- 3 full-time Employees Minimum Enrollment Required
- Choose Any Dentist
- Immediate Coverage for Preventive and Restorative Procedures
- Can be used for Section 125 Cafeteria Program
- Rates are guaranteed for 1 Year from the Group's Effective Date
- Coverage Available: Employee Only, Employee & One Dependent (Spouse or Child), or Family Coverage.
- Maximum Deductible per Family $150.00
- Annual Open Enrollment Period
Individual Dental
- Choose any dentist
- Issue ages 18 - 65
- Optional spouse and dependent coverage
- Orthodontia benefits for eligible dependent children
- Billing choices: monthly Visa/MasterCard or bank draft or annual direct bill
